Financial Disclosure Requirements
 
District law (DC Official Code §1-1106.02) requires members of certain boards and commissions to file an annual financial disclosure statement with the Office of Campaign Finance (OCF Form 62):
  1. On or before May 15 after every calendar year in which they serve on a board or commission; and
  2. Within 90 days of the end of their appointment, if the member ceases to serve on a board or commission prior to May 15 of any year.
In addition, a board or commission member must notify the Office of Campaign Finance and the Office of Boards and Commissions of any pertinent changes in status that the board member may experience after he or she has filed the annual financial disclosure statement, including a change of address, must be filed within 30 days of that change.
 
The Office of Campaign Finance is not required to send notices or forms to affected board members as a reminder. The Office of Boards and Commissions does provide financial disclosure statement forms to members when they are sworn in to serve and may send reminders to members. Members may obtain a copy of the form from the OBC website, the Office of Campaign Finance website, by requesting a form be mailed or faxed, or in person at the Office of Boards and Commissions at One Judiciary Square; 441 4th Street, NW; Suite 530-South; Washington, DC 20001.
The financial disclosure statement must be notarized. The Office of Boards and Commissions will provide notary services at no charge to a member of a board or commission. These services are available on a walk-in basis.
 
The financial disclosure statement must be filed directly with the Office of Campaign Finance. The Office of Boards and Commissions will not accept, and will not file, a financial disclosure statement form on behalf of an individual.
